In the photo – the team of the Primary Healthcare Center of Pervomaisk, who provide daily care to patients in their community. They, more than anyone else, understand that the health of the community is one of its most valuable assets.
To preserve this "asset," the community of Pervomaisk in Kharkiv region has joined our "Healthy and Capable Community" project, with the support of ULEAD with Europe.
We often receive patients in advanced stages of illness, who require long-term and expensive treatment. We can only compensate for severe pathologies to slightly improve their quality of life. But this is not how it should and can work if we want to preserve our human capital

In 2019, Mykola had the opportunity to visit Kinzigthal and see how the integrated service delivery model works. The community is already forming a working group that, together with experts from our Foundation, OptiMedis, and the Kharkiv Expert Group on Health Reform, will learn about integrated service delivery models and working with data needed for a comprehensive assessment of community health management needs.
We want to show our residents that integrated care is not some special program or manipulation performed by a nurse in the hospital. In fact, it’s about the community taking responsibility for the health of its residents

A memorandum of cooperation with "Health Solutions" has been signed – marking the start of our joint efforts to move the Pervomaisk community towards the status of a "healthy" community even faster, which we will continue to support.
