The foundation of the project – the integrated service system model from the town of Kinzigtal, Germany
This system integrates the local network of family doctors Ärztenetz-MQNK and the healthcare management company OptiMedis, which together launched a program that helps residents access medical and other services for health care while reducing treatment costs.
With the support of ULEAD with Europe, the team of experts from OptiMedis and the Kharkiv Expert Group for Healthcare Reform Support are working on implementing this model in Ukrainian communities, particularly through our "Healthy and Capable Community" project.
How the Program Works in Kinzigtal

The program covers the entire population of the Kinzigtal region and currently helps over 30,000 people. Every resident can voluntarily participate in the program and actively engage in the decision-making process regarding their health.
The program has been continuously evaluated both internally and externally since its implementation in 2005, with positive trends and successful outcomes.
Key Principles of the "Healthy Kinzigtal" Program
- Community-Centered and Long-Term Focus
The "Healthy Kinzigtal" program focuses not just on providing medical services, as is common, but on improving overall health at all levels. It aims to protect the community's health in the long run and reduce treatment costs. - Prevention as the Core Focus
The program supports prevention, self-management, patient care, and the use of electronic health networks to deliver services. This approach reduces the strain on the healthcare system and ensures effective resource utilization. - Patient Involvement in Decision-Making
The program includes individualized service plans and agreements on shared goals between doctors and patients. This allows patients to manage their health and take an active role in medical decisions.
